Background

Medication non-adherence remains the leading contributor to relapse and hospitalization in individuals with psychotic disorders (1). Consistent adherence to antipsychotic therapy is critical for sustained symptom control, yet real-world adherence rates remain suboptimal. Long-acting injectable antipsychotics (LAIs) offer a strategy to mitigate this challenge, particularly formulations with extended dosing intervals. Despite their benefits, LAI utilization in Canada has historically been lower than in other regions (1).

Objectives

To describe early clinical experiences, practical considerations, and real-world outcomes associated with the use of aripiprazole extended-release injectable suspension administered once every two months (Abilify Asimtufii®) in an outpatient Canadian setting.

Methods

This poster presents observational insights from clinical use in a Canadian outpatient mental health centre. Key domains examined include patient selection criteria, dosing and administration considerations, cost and access factors, and adherence-related outcomes. Outpatient data set will be presented. A representative case study is included to illustrate clinical decision-making and patient response.

Results

The introduction of Abilify Asimtufii® has expanded treatment options for adults with schizophrenia and bipolar I disorder (2,3). Its extended dosing interval (six injections annually) offers a meaningful reduction in treatment burden, which may improve adherence in appropriately selected patients. Clinicians identified several important considerations, including prior stabilization on oral or injectable aripiprazole, monitoring during transition phases, and navigating reimbursement pathways. Early observations suggest improved patient satisfaction and engagement, particularly among individuals with a history of partial adherence. Cost and formulary access remain notable barriers in some cases.

Conclusions

Abilify Asimtufii® represents a promising advancement in LAI therapy by reducing injection frequency while maintaining therapeutic continuity. Thoughtful patient selection, proactive management of logistical barriers, and clinician familiarity with dosing protocols are essential to optimize outcomes. Broader uptake of extended-interval LAIs may contribute to improved adherence and reduced relapse rates in Canadian clinical practice. The need for education of physicians on this topic, as well as advocacy in switching considerations to a longer interval treatment, is advised.
